Population Health, Next Steps: A Roadmap 

Building a healthier Kentucky will depend largely on what we do beyond the healthcare system. Population Health or total population health is defined as “the health outcomes of a group of individuals, including the distribution of such outcomes within the group.” The meeting will highlight current successful national, state and community efforts to form partnerships to improve health outcomes.
